Request Archived Health Records
If you need a copy of your health records, your local military hospital or clinic can provide one. You can only get copies of records related to a hospitalization, fetal tracings, or ambulance services (collectively referred to as “inpatient records”) from the facility where you got this care. Be prepared to give your full name, sponsor’s Social Security number, DoD ID number, and place and dates of your last treatment information.
